Vaccination Form Submission

All attendees to a Dystopia Rising New Jersey event must submit documentation via email to show that they are vaccinated with the appropriate wait period following their final vaccination, as well as proof of a booster or a negative test taken 72 hours before an event.

The subject of your email for vaccine submission should be the following format:
”Name Player # Vaccination Card Submission”

Ex: “DJ Martin 9999 Vaccination Card Submission”

The email should contain a picture of your official issued proof of vaccination (Vaccination card, Excelsior Pass, etc) with your name and date of vaccinations clearly visible.

For submitting your booster information or for sending a test result before game, send an email containing the related documentation with the subject as “Name Player # Booster Card Update” or “Name Player # Negative Test Result Date” via email.

Ex: For Boosters “DJ Martin 9999 Booster Card Update”
Ex: For 72 hour test “DJ Martin 9999 Negative Test Result 2/17/22”
